If it looks like yeast then use on over the counter l yeast cream. If not just use Vaseline, nothing else. Diaper rash cream, and antibiotic creams may be irritating his delicate little skin. And yes mention this to the doc, if it doesn't go away in a couple days take him some where else. Good luck!
